APPLYING FOR SERVICES
Thank you for your interest in our Adult Day Health Program.
There are two ways that you can apply for entrance to our program; online or
in person. Please do not hesitate to call (404) 499-2755 during business
hours for questions you may have.
Persons desiring to participate in our program should complete both the
Participant Information Form and Physician Exam Form and submit to our
center. The forms can be viewed and printed online by clicking on the
highlighted links above on this page. If you are unable to open the file,
please click the following link:
Once completed, return the forms to our center either by mail or in person.
You will be contacted as soon as possible thereafter.
